Our website questionnaire asks about your age band and a few questions about what you would like help with. Responses provided prior to account creation will not be used as part of your medical assessment. These answers are pre-account and non-clinical, are held in your browser session only, are not stored on our servers, are not seen by a clinician, and are never shared into a medical record. If you choose to create an account, the medical intake starts fresh.
If you join our list we store your email, first name if given, state and the program you were interested in, to tell you when we open. You can unsubscribe at any time.
Once you create an account, health information you provide is protected health information handled under our Notice of Privacy Practices and HIPAA.
